| I have indicated my choice of environmental education programs using the “Program Selection Menu ” | |
| I have been contacted by Alexis Sohlden to confirm and finalize our scheduled program at LAC | |
| I have completed and returned my LAC contract with my P.O. or deposit | |
| I have chosen and contacted any necessary chaperones to maintain the necessary 1:10 adult: student ratio | |
| I have given all adults a copy of the “Information for Leader(s) and Chaperone(s)” document | |
| I sent out a “Permission Letter ” to all participants (and parents if participants are minors) | |
| I have given all parent(s)/guardian(s) a copy of the “Information for Parent(s) and Guardian(s)” document | |
| I have had all participants sign a “Consent Form” prior to our LAC visit | |
| I have made arrangements for transportation and have communicated “Directions to LAC” | |
| I have discussed materials that all participants should bring using the “Suggested Packing List” | |
| I have made nametags for all participants and adults (red borders for those with a “no-publicity” preference) |
